That adaptation is the reason elite endurance athletes train at altitude. It is also the reason some researchers have asked whether people who live high up age more slowly — a question that has produced unusually good data over the past decade.

To see how altitude fits alongside the standard mortality inputs, our longevity age calculator lets you weigh lifestyle factors against each other in one place.

What Living at Altitude Does to the Body

The core change is chronic mild hypoxia — lower oxygen availability in every breath. The body compensates through a well-documented cascade:

Ventilation increases immediately. At 2,500 m, the partial pressure of oxygen falls to roughly 74 percent of sea-level values, and breathing volume rises to compensate.

Erythropoietin (EPO) surges within 6 to 24 hours, signaling the bone marrow to produce more red blood cells. Hematocrit typically rises 3 to 8 percent over the first month.

Capillaries proliferate in muscle tissue over 4 to 12 weeks, improving oxygen delivery efficiency at the tissue level.

Mitochondrial density increases in skeletal muscle with sustained exposure, a change associated with improved metabolic efficiency.

These are the same adaptations that make altitude training camps effective for runners and cyclists. The open question is whether they translate into slower aging across a whole lifetime.

The Evidence from Populations Living High

Two populations dominate this research: residents of the Bolivian and Peruvian Andes, and the long-lived communities of Sardinia’s mountainous interior and the Greek island of Ikaria. Both sets of data are messy, but a pattern has emerged.

Population / study

Altitude

Key finding

US county-level mortality analysis (2010s)

1,500–3,000 m

Lower age-adjusted mortality than sea-level counties after income adjustment

Andean highlanders (Bolivia, Peru)

3,500–4,000 m

Reduced cardiovascular mortality; higher rates of chronic mountain sickness in some cohorts

Sardinian mountain villages

600–1,000 m

Male centenarian rate roughly double the regional average

Summit County, Colorado

~2,800 m

Lowest all-cause mortality of any US county in repeated analyses

The US county data are the most striking. Summit County, Colorado — average elevation around 2,800 meters — has repeatedly recorded the lowest all-cause mortality rate of any county in the United States. Similar results appear across high-elevation counties in Utah, New Mexico, and Wyoming.

Why the Correlation Is Hard to Read

Mountain communities differ from lowland ones in ways that have nothing to do with oxygen. They tend to be wealthier, more physically active, less likely to smoke, and more socially connected. Summit County is a ski resort economy with high incomes and an outdoor-recreation culture — factors that independently predict long life. Untangling altitude’s own contribution requires statistical adjustment, and the adjusted effect, while still present, is considerably smaller than the raw numbers suggest.

There is also a genuine counterweight. Chronic hypoxia is not harmless. Long-term residents above 3,500 meters show elevated rates of chronic mountain sickness, higher pulmonary artery pressure, and in some Andean cohorts a modest increase in stroke risk. The relationship appears to be U-shaped or at least plateaued: moderate altitude may be protective, extreme altitude is not.

A Worked Comparison

Two men, both born in 1966 and therefore turning 60 this year. One has lived his entire life in Denver (1,600 m). The other lives in a Florida coastal town at sea level. Both are non-smokers with similar incomes and both walk 7,000 steps a day.

If you run each through a functional age assessment — resting heart rate, grip strength, blood pressure, VO2 max estimate — the Denver resident will typically show a slightly higher VO2 max and lower resting heart rate for the same training volume, consistent with mild altitude acclimatization. That might translate to a biological age estimate two to four years younger. But swap in a Denver resident who drives everywhere against a Floridian who swims daily, and the advantage reverses completely. Altitude is a modifier, not a dominant variable.

For a sense of which levers carry the most weight, our fitness age calculator isolates the cardiorespiratory inputs that altitude partly influences.

Frequently Asked Questions

Does moving to altitude in midlife extend lifespan?
There is no evidence that relocating as an adult confers the same mortality profile as lifelong residence. Most of the observed benefit in high-elevation counties tracks with lifestyle and income rather than acclimatization acquired late.

Is altitude training the same thing?
No. Altitude training improves endurance performance through the same EPO and capillary adaptations, but the exposure is weeks long and performance-focused. Longevity effects, if any, come from decades of residence.

What is the optimal elevation?
Research suggests the protective signal is strongest between roughly 1,500 and 3,000 meters, with benefits flattening and risks rising above 3,500 meters. There is no experimental evidence for a precise optimum.

Should I worry about chronic hypoxia if I live high?
Above 3,500 meters, yes — periodic screening for polycythemia and pulmonary hypertension is reasonable. Below 3,000 meters, the physiological burden is generally well tolerated by healthy adults.

The Bottom Line

People who live at moderate altitude do appear to live somewhat longer on average, but the honest read of the data is that most of that advantage comes from the lifestyle that tends to accompany mountain living — daily movement, clean air, lower smoking rates, tighter communities. The hypoxia-driven adaptations are real and measurable, and they probably contribute a modest additional benefit. If you want to capture some of it without moving, the leverage is in the behaviors: consistent aerobic training produces many of the same cardiovascular adaptations as altitude acclimatization.
