The loss is not spread evenly across the body or the decades: almost none of it happens before 40, and the steepest drop occurs after 60 when spinal discs compress and vertebral bone density falls.

The measurement matters clinically. A height loss of more than 4 centimetres above the age-adjusted average is one of the earliest screenable indicators of vertebral fracture risk, which is why doctors now measure height at annual physicals. You can place your own trajectory against the typical curve with our height growth predictor.

A Decade-by-Decade Average

Age range

Average loss per decade (men)

Average loss per decade (women)

Main mechanism

20-40

0.0 cm

0.0 cm

Peak height, no measurable decline

40-50

0.3 cm

0.5 cm

Early disc dehydration

50-60

0.7 cm

1.2 cm

Disc compression, posture change

60-70

1.2 cm

2.0 cm

Vertebral bone loss accelerates

70-80

1.5 cm

2.5 cm

Osteoporosis, kyphosis

80+

1.5-2.0 cm

2.5-3.0 cm

Cumulative vertebral deformity

The totals stack to the widely cited figure of 2.5-3 cm for men and 5 cm for women by age 80. Anyone measuring outside these bands by more than 2 centimetres is outside the normal distribution and worth a bone density scan.

The Three Mechanisms, in Order of Contribution

Intervertebral disc thinning — about 40% of total loss. Each of the 23 discs in the spine loses water content with age. At 20 a disc is roughly 80% water; at 70 it is closer to 65%. Twenty-three discs each losing 1-2 millimetres adds up to 3-4 centimetres on its own.

Vertebral compression — about 45% of loss in women. Bone mineral density peaks around 30 and falls roughly 1% per year thereafter, accelerating to 2-3% per year in the five to seven years after menopause. Compression fractures of the vertebrae can each remove up to 2 centimetres instantly.

Postural change — about 15%. Thoracic kyphosis curves the upper spine forward, which reduces standing height without any change to the bones or discs themselves.

A Worked Example

A woman measured at 168 cm on her 25th birthday and 163 cm at 75 has lost 5.0 cm over 50 years — squarely on the population average. Breaking it down against the table: 0.5 cm in her forties, 1.2 in her fifties, 2.0 in her sixties, and 1.3 in the first half of her seventies. Each individual decade looks negligible; the sum does not.

If instead she had measured 165 cm at 25 and 163 at 75 — a 2 cm loss over 50 years — that would be unusually low, and usually indicates a measurement taken with different posture, at a different time of day, or with the shoes on. Self-measured height varies by up to 2 centimetres between morning and evening because discs re-expand overnight.

Measurement Traps That Produce False Loss

Before concluding anything about your own rate of loss, eliminate these four sources of error:

Time of day. Measure in the morning, consistently. Evening measurements can be 1-2 cm shorter after a day of gravity loading.

Footwear. A 1 cm heel is a 1 cm error. Height is defined as barefoot.

Head position. The Frankfurt plane (eye socket and ear canal level) is the clinical standard. Tilting the chin changes the reading by up to 1.5 cm.

Wall calibration. A pencil mark on drywall is not a stadiometer. A wall-mounted tape with a spirit level gives repeatable results within 0.2 cm.

Frequently Asked Questions

Can height loss be reversed? Postural and disc-related loss can be partially recovered through decompression and core work, typically 1-2 cm. Bone-related loss from compression fractures cannot be reversed, only halted.

At what point is height loss a medical concern? More than 4 cm below the age- and sex-adjusted average, or any loss of 2 cm or more within a single year, warrants evaluation for vertebral fracture.

Does the same thing happen to other body parts? Different systems peak at different ages.
