A 5&#8217;4&#8243; woman with a 34-inch waist sits at 0.53, above the line. A 6&#8217;2&#8243; man with the same 34-inch waist sits at 0.44, comfortably below it. Waist-to-hip ratio requires different thresholds for men and women and performs worse in predicting cardiovascular events in several large cohort studies.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The 0.5 cutoff is also easy to act on. If you measure your raw waist circumference and your height in the same units, the rule is: if the number is more than half your height, you&#8217;re carrying a level of central fat associated with elevated risk.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Is BMI Useless Then?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">No, but it&#8217;s a screening tool, not a diagnosis. BMI&#8217;s failure mode is specific: it misclassifies muscular people as overweight and misclassifies people with low muscle mass and normal weight as healthy. A 5&#8217;10&#8221; athlete at 200 pounds reads BMI 28.7, in the &#8220;overweight&#8221; band, while carrying 12% body fat. The same BMI in an office worker with 30% body fat means something very different.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For population-level screening it works fine because those errors cancel out across thousands of people. For an individual, pair it with a waist measurement. If BMI says 27 and waist-to-height says 0.44, the BMI number is probably the misleading one.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">A Worked Example<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Take a 41-year-old woman, 5&#8217;6&#8243; (66 inches), 150 pounds (68 kg).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>BMI:<\/strong> 68 &divide; (1.68)&sup2; = 68 &divide; 2.82 = <strong>24.1<\/strong> \u2014 normal range<\/li>\n<li><strong>Waist:<\/strong> 31 inches. <strong>Waist-to-height:<\/strong> 31 &divide; 66 = <strong>0.47<\/strong> \u2014 below the 0.5 line<\/li>\n<li><strong>Hip:<\/strong> 40 inches. <strong>Waist-to-hip:<\/strong> 31 &divide; 40 = <strong>0.78<\/strong> \u2014 well below 0.85<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">All three agree here. Now change one input: the same woman with a 35-inch waist. BMI is unchanged at 24.1, still &#8220;normal,&#8221; but waist-to-height jumps to 0.53 \u2014 above the threshold. This is the scenario BMI misses most often: normal weight, centrally distributed fat.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Frequently Asked Questions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Where exactly do you measure the waist?<\/strong> At the midpoint between the lowest rib and the top of the hip bone, after exhaling normally. Not at the navel and not at the narrowest point.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Do smart scales give usable body fat numbers?<\/strong> For tracking direction over months, yes. For absolute accuracy, no. Bioelectrical impedance varies with hydration \u2014 a reading taken after a workout can differ by several percentage points from one taken first thing in the morning.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Which single metric should I track if I only track one?<\/strong> Waist-to-height ratio.
